1. Introduction
At Whitchester House, honesty is a pillar of recovery. The Duty of Candour ensures that if an unexpected mistake occurs, we are open, provide an apology, and learn from it.

2. Incident Summary
During this period, Whitchester House reports zero (0) incidents where the Duty of Candour was triggered. We are pleased to confirm a Nil Return for the 2025/26 cycle.

Physical or Structural Harm (Injury requiring extra medical treatment): 0


Psychological or Functional Harm (Long-term distress or loss of function): 0

3. Our Commitment to Safety
While there were no situations to report, we remain proactive through the following measures: 
Staff Awareness: Every staff member is trained to identify and report unintended harm immediately.
Transparency: We provide an environment where "near misses" are discussed openly in team meetings to prevent future issues. 


Support: Our Dedicated Family Liaison ensures families are kept informed, maintaining the trust essential to our supported housing/recovery model.
